How to Compare D-SNP Plans

Choosing among D-SNPs requires comparing more than just premiums. This guide walks through the key factors to evaluate before enrolling.

Educational information only. SubsidyCheck does not determine eligibility, enroll you in a plan, or act as a government agency. Final decisions are made by Medicare, your state Medicaid agency, and licensed plan administrators.

Start With Your Must-Haves

List your current doctors, specialists, hospital, and pharmacy. A plan that does not include your providers or medications may cost you more in inconvenience and out-of-pocket expenses—even if it offers richer supplemental benefits.

Note any chronic conditions requiring regular specialist care. Plans with strong disease management programs for your conditions may provide better support.

Compare Provider Networks

Use each plan's online provider directory to verify network participation. Call your doctors' offices to confirm—they may have more current information than online directories.

Consider network size, geographic coverage (especially if you travel or split time between locations), and whether the plan is an HMO (referrals required) or PPO (more flexibility).

Review Drug Formularies

Check that all your medications are on the plan's formulary and note their tier levels. Higher tiers mean higher copayments. Look for prior authorization or step therapy requirements that could delay access.

If you have Extra Help, your copayments are capped—but the drug must still be on the formulary. Mail-order options may offer convenience and savings.

Evaluate Supplemental Benefits

Compare dental, vision, hearing, OTC, transportation, and fitness benefits. Focus on benefits you will actually use—a large OTC allowance matters less if you need denture coverage.

Check annual limits, network requirements, and whether unused benefits roll over. Read the fine print in the Summary of Benefits.

Consider Plan Quality and Support

Medicare Star Ratings (1–5 stars) reflect quality scores for member experience, health outcomes, and customer service. Higher-rated plans are not automatically better for everyone, but ratings provide a useful data point.

Ask about care coordination availability, member services hours, and language support. Talk to current members if possible, though individual experiences vary.

Making Your Decision

Create a simple comparison chart with your top two or three plans. Weigh network and formulary fit against supplemental benefits and ratings. There is rarely one perfect plan—the goal is the best overall fit for your situation.

SubsidyCheck provides educational tools to help you prepare comparisons. Final enrollment decisions should be based on official plan documents and, when helpful, guidance from SHIP counselors.

Frequently asked questions

Should I choose the plan with the most supplemental benefits?

Not necessarily. Network and formulary fit often matter more. A plan with fewer extras but your doctors and drugs covered may serve you better.

How do Star Ratings work for D-SNPs?

D-SNPs receive Star Ratings like other Medicare Advantage plans. Ratings are published on Medicare.gov and updated annually.

Can I try a plan and switch if I do not like it?

Dual-eligible members can use the quarterly SEP to switch once per quarter. Take advantage of this flexibility if a plan does not meet expectations.
